The 8.5×11 inch pdf includes 12 editable tags. Each tag measures 2.25×2.25 inches.

After downloading the pdf, open it up in Adobe Acrobat. You should be able to see a blue text box where you can type in your child’s name.

Once your tags are personalized, simple print and cut along the guidelines. For the best quality, I would highly recommend printing on a bright white cardstock.
