I always loved that my mom kept so many of the pictures and projects from when I was younger. It is so much fun looking back on everything and seeing how I changed over the years.
Now that I am a mom myself I love doing the same for my kids. One tradition I’m excited about starting is having my kids fill out a first day of school questionnaire every year. I plan on placing them in a folder or binder and keeping it for us to look back on over the years and see how they changed.
This free printable questionnaire is an easy and fun tradition you can start for your kids as well! Featuring a variety of questions to answer along with a spot to draw or glue a picture, it is a great way to start off each new school year.
The questionnaire features the following questions:

- Age
- Best friends name
- Hobbies
- Favorite school subject
- Favorite food
- Favorite color
- Favorite TV show
- Favorite song
- What they want to be when they grow up
In the box in the bottom right corner your child can either draw a picture of themselves or you can take a photo and tape or glue it to the page.
